Download and untar monobook to wikiroot/lib/tpl/

Go to Admin section and select monodoku as wiki template.
Download and install indexmenu plugin.
The indexmenu plugin will display file folder icons in our index in place of plain vanilla links with arrows. Next, we'll choose what set of icons to show. From the Toolbox navigation bar, select Admin, followed by a new link called Indexmenu Utilities under a section, “Additional Plugins”. You'll be shown a list of Themes. The first is your site's default theme. Click the Show button next to the second one – the URL with the author's site name. Dokuwiki will connect to the site and show you a preview of the choices. I picked “bj-tango”. You can pick more than one set. Click the Install button next to each one you want to retrieve.
As mentioned above, you typically use a plugin by placing a call to it on a wiki page. The indexmenu plugin, however, is different. It has to be triggered on all of our wiki pages, because we're always going to show the navigation index. The details can be found on the indexmenu page.
Create a new file called navigation.txt in the root of our wiki documents.
Monodoku use 'wiki:navigation.txt' as the index file. This file should be created so that the menu is display properly:
cd /var/www/dokuwiki cd data/pages touch navigation.txt
Edit this new file navigation.txt and type one line (replace “bj-tango.png” with the name of your preferred icon.):

Step 1: Make a backup of your DokuWiki, this is not necessarily needed but recommended
Example:
tar zcpfv dokuwiki-backup.tar.gz /path/to/dokuwiki
Step 2: Read the changelog to see if there are any special things to consider
Step 3: Download the newest release from the download site.
cd ~ wget http://.../dokuwiki-xxxx-xx-xx.tgz
Step 4: Unpack the distribution tarball and upload/copy the files over your previous installation. Make sure to overwrite all files when prompted.
tar zxvf dokuwiki-xxxx-xx-xx.tgz 'cp' -af dokuwiki-xxxx-xx-xx/* /path/to/dokuwiki/
The quotes on cp assure that it will run as is, even if an alias is set.
Check copied files ownership and permissions
chown -R apache:apache
Step 5: Delete all unused files
Step 6: Upgrade your template and plugins if necessary
Step 7: Wait for a day for update notifications to go away, or speed the process by following the steps in Update Check
